Transitioning into Adulthood
Trellis also provides Adult Life Planning (ALP) to support those with developmental disabilities as they transition to adulthood, find resources, and develop skills for work and daily life. Since 2022, Trellis’ Adult Life Planning Services have assisted more than 130 families, providing them with the essential tools and guidance for effective adult life planning. As one of just a handful of providers with a full-time adult life planner in Maryland, our families receive consistent, expert guidance, making their transition smoother and more structured.
What is Adult Life Planning (ALP)?
Adult Life Planning (ALP) is a comprehensive service that helps people with autism and their families navigate the transition to adulthood. Designed for those ages 14 to 26 who participate in the Maryland Autism Waiver, ALP guides them in setting goals, creating a plan, and receiving ongoing support. This service is provided virtually, giving parents 20 hours per year to help in transition planning for their young adult.
To accommodate different schedules and locations, we primarily meet with families online. Our services are flexible and personalized, offering customized meeting times and tailored plans to meet each family’s unique needs.
The Referral Process
How to Get Referred to Adult Life Planning Services
Adult Life Planning (ALP) is a billable service available only to clients enrolled in the Maryland Autism Waiver. To request ALP services, families should contact their assigned Autism Waiver Service Coordinator and ask to have Adult Life Planning added to their Plan of Care.
